Two IACUC personnel policies may require action for each person named on an IACUC application before the IACUC will grant approval. Details for each of these requirements can be found below:
ACUOHP CLEARANCE REQUIREMENT:
UCSC policy requires that all faculty, staff, visiting researchers, and students who work directly with vertebrate animals, unfixed animal tissues or body fluids, and/or who work in animal housing areas, participate in the Animal Care and Use Occupational Health Program (ACUOHP). The ACUOHP provides a framework for assessing and controlling potential health risks for participants in animal research protocols. Your IACUC protocol will not be approved until ALL personnel have completed this requirement. ACUOHP clearances are granted on a "per protocol" basis. This process may take several weeks so do not delay in submitting the forms for all personnel named below.

IACUC's ONLINE TRAINING REQUIREMENT:
Online IACUC training is required for all personnel listed on a UCSC IACUC application before the IACUC will give approval.
IACUC online training for Full Use (CONTACT) IACUC Applications ONLY: (including amendments to Full Use-Contact protocols)
REMEMBER: You do NOT need to take the Human Subjects, Primate or Dog modules.
IACUC online training for non-contact IACUC Applications ONLY (i.e., Observational and/or Vertebrate Tissue Sample): (including amendments to non-contact protocols)

- Complete the modules in the "Laboratory Animal Welfare, Basic Course" Gradebook with the EXCEPTION of the following:
- A minimum average (combined) score of 60 for all required CITI modules is required to pass IACUC training requirement, i.e., ?60% average score to pass.
If you are not sure whether you are already signed onto the site, if you can't remember your user name or password, if you are not sure which course you need to take, or if you have a problem logging on or taking the courses, contact the IACUC Coordinator (carc@ucsc.edu or 9-3150).
Here are the instructions for signing up for the CITI website training program that is specific to researchers who will be working with animals. Be sure to follow the instructions carefully to insure that you complete the correct course or courses. If you have any questions, contact the IACUC office at 459-3150.
To access the training:
* You should go to http://www.citiprogram.org/ to register for CITI online training.
* Once there, simply click on "New Users Register Here".
* Under "Select your institution or organization" page select "University of California, Santa Cruz" in the "Participating Institutions" drop down box.
* Next you should proceed to create your own username and password.
* Next you should click on "Laboratory Animal Welfare"* Under 'Role in Human Subjects Research' select your role (e.g. PI, Student Researcher, Vet). After going through registration process you should be set up as a CITI Learner.
* When you enter the program, check both boxes that say IACUC Investigators (Question 1, Groups 3 and 4)
* On Question 3, please check "Working with the IACUC Course." You also have the option of selecting animal specific courses, if you are interested.
* When you enter the IACUC training course, you need to visit the required section and complete all the elective modules except: 'Dog Exercise', 'Primate Psychological Enrichment' and 'Using Human Patient Care Areas for Animal Research'.

"Working with the IACUC" This is the course that IACUC requires for approval of all protocols.
"Essentials for IACUC Members" This course should be taken by all members of IACUC.
On a case by case basis, other courses may be required by either IACUC or the Vivarium staff for personnel working in the Vivarium.
